Individual therapy is offered for adults ages 18 years and up.

For your convenience all of our forms are electronic. Please click the link below to request a new patient portal to support scheduling, complete your new client paperwork, sign consent forms, and appropriate waivers. The links are HIPAA compliant and are reviewed directly by office staff once completed.

Currently Dawn is the sole provider and accepts most major insurance companies and plans. If you do not have insurance or would prefer to pay out of pocket, there is a competitive rate of $125 for 1-hour sessions.
